Note: There is a $75 re-application deposit that must be paid for this form to be considered complete. This deposit is non-refundable unless your application is refused for any reason by the school.

Note: This form and the $75 deposit must be received at the school office ON or BEFORE the date of JULY 15. If received AFTER JULY 15, the deposit will not be credited to your account and will be charged as an additional fee. The Early Re-application Scholarship can be applied for only if this form and the deposit have been received by our office ON or BEFORE MAY 31st.
